Overview Developmental Disabilities Institute, DDI, is one of the largest providers of care to children and adults with Autism, developmental disabilities and other special needs. DDI has been a part of Long Island for over 50 years serving over 1,500 children and adults with Autism, other developmental disabilities and special needs. DDI provides innovative and expansive services to our population through various residential services, day habilitation programs and vocational services. Rate of pay will be determined by the actual service budget set for each participating individual. Responsibilities Provides one-on-one assistance to the individual in the home and in the community to enhance a variety of life skills to include social, recreational and any other activities of daily living.Complete training to use DDI billing softwareAssists the individual in identifying social, recreation and other community resources the individual may wish to access to increase skills and independence.Regularly ensuring the submission of timesheets, mileage sheets, monthly expense reports, and service notes/documentation of the services provided are completed in a timely manner.Enhances and promotes social interaction and community participation.Provides transportation to the individual to and from community activities as needed.Facilitate a mentoring relationship/companionship to assist in building social and communal relationship.Assist, ensures and promotes necessary techniques/materials in order to teach the individual all aspects of daily skills that pertain to the individuals valued outcomes in the community habilitation plan. In cooperation with the individual, family, Circle of support, MSC, the community habilitation staff will assist in the identification and development of valued outcomes.Complete all trainings, in-service workshops and recertification trainings as required.Qualifications HS Diploma required Excellent organizational, verbal and written communication skills and interpersonal skills.Must have own transportation for travel between work locationsWorking Knowledge of OPWDD services and supportsValid NYS Drivers License
