Johnston Memorial Hospital Registered Nurse (RN)-BPCI-A Navigator-Care Management-Full Time in Smithfield, North Carolina

This position serves as the point person for the Bundled Payments for Care Improvement Advanced (BPCI-A) program operational workflow within the Care Management department. The Navigator is responsible to drive the culture change regarding transition planning, and is accountable for optimizing BPCI-A performance metrics and observation conversion rates. This position ensures the timely and thorough utilization review for all hospitalized patients in accordance with the Hospital’s Utilization Management Plan.

MINIMUM QUALIFICATIONS:

Graduate of an approved school of nursing. Bachelor’s degree in nursing (BSN) preferred. Licensed to practice as a Registered Nurse in the state of North Carolina. A minimum of two (2) years of experience in case management, utilization management, discharge planning and/or care coordination. Certification in Case Management (ACM, CCM) preferred.
