Home Care Physical Therapist Position Purpose:

Provides professional physical therapy services to the organization’s clients as prescribed by the physician, and in compliance with the state’s professional practice act for physical therapy, any applicable licensure/certification requirements, and the organization’s policies and procedures.

What You Will Do:

  • In collaboration with Team Leader or designee, effectively and efficiently prioritizes visit content and plans visits based on client capabilities, condition, preferences (when possible), physician orders and available resources.

  • Maintains a working knowledge of conditions of participation and applicable federal, state and local laws and regulations, the Organizational Integrity Program, Code of Ethics, as well as agency policies and procedures and adheres in a manner that reflects honest, ethical and professional behavior.

  • Accurately assesses clients at admission and other required time points, and utilizes appropriate standardized documentation. Assesses the client’s condition and needs and determines the agency’s ability to meet those needs in the home care setting.

  • If therapy only case, informs the client/caregiver about the agency, care/services available, client obligation for payment, and rights and responsibilities before care is initiated.

  • Develops appropriate assessment-based plan of treatment with physician and client. Plan includes attainable episode goals and agency services needed to improve and/or stabilize client’s medical condition and promote independence. Participates in timely revision of the plan of treatment as appropriate.

  • Makes appropriate referrals for evaluation/care to other disciplines and services, and coordinates care with others to ensure effective and efficient care is provided.

  • Utilizes interview, observation and evaluation in assessing clients and applies professional judgment, consistent with practice standards, in formulating therapy interventions and making recommendations to the physician and client.

  • Reports changes in client condition as appropriate and in a timely manner, to the client’s physician and obtains orders for changes in the plan of treatment.

  • Initiates and implements appropriate preventive, therapeutic and rehabilitative procedures and techniques to clients.

Minimum Qualifications:

  • Possession of a degree, which includes an internship from a baccalaureate/university program in physical therapy approved by the American Physical Therapy Association.

  • Licensed to practice as a physical therapist in the state of Maryland.

  • Must have a minimum of one (1) year experience as a physical therapist.

  • Current working knowledge of conditions of participation and home healthcare experience preferred.

  • Must have current Driver’s license and reliable transportation
