Hackensack Meridian Health Clinical Therapist II (Addiction Recovery Services) - Full Time in Shrewsbury, New Jersey

The Clinical Therapist II treats patients by utilizing a variety of modalities as a member of the multidisciplinary clinical treatment team. Also provides psychological/social interventions that support patients and families relating to behavioral health, levels of care, hospitalization, disease and illness and wellness and recovery.

Responsibilities

A day in the life of a Clinical Therapist II at Hackensack Meridian Health includes:

  1. Provides a comprehensive assessment of a patient's psychosocial needs, develops treatment plans and treatment plan reviews based on findings and according to department policy.

  2. Provides effective behavioral health treatment and education to patients, whether including assessment or therapy (individual and/or group), and shares and interprets information to staff, families, and/or other approved agencies.

  3. Completes all documentation written reports in a timely fashion in accordance with department and hospital policy. as required by the Medical Center or Agency policy.

  4. Provides services to patients in accordance with Licensure regulations defined by the respective licensure board, i.e., ASWB or NBCC.

  5. Utilizes and obtains and interprets information in terms of patient needs, knowledge of growth and development, and an understanding of the evidence- based treatment options available range of treatment required for specific populations that age groups, when developing treatment plans.to develop a treatment plan specific to patient needs.

  6. Provides comprehensive case management services/assessment as warranted.

  7. Assess Social Determinants of Health (SDOH) and provide information, education and/or referrals as appropriate to meet identified patient needs.

  8. Collaborates with the patient (with their consent), family, and other support persons involved in the patient's care to develop a comprehensive discharge plan that best meets the continuing care needs of the patient.

  9. Reassesses treatment plan on an ongoing basis, evaluates progress, and revises plan when clinically indicated.

  10. Maintains protected patient health information as required by law, State Licensing, TJC and HMH policy.

  11. Assists other behavioral/medical team members in responding to emergencies.

  12. Has a working knowledge of various community agencies and resources and refers patients when appropriate.

  13. Assists with interview and orientation of new staff members.

  14. Supervise social work/counseling students completing field placement.

  15. Identifies the needs of the patient population served and modifies and delivers care that is specific to those needs (i.e., age, culture, language, hearing and/or visually impaired, etc.). This process includes communicating and providing instruction/education with the patient, parent and/or primary caregiver(s) at their level (developmental/age, educational, literacy, etc.).

  16. Lifts a minimum of 5 lbs., pushes and pulls a minimum of 5 lbs. and stands a minimum of 4 hours a day.

  17. Adheres to the standards identified in the Medical Center's Organizational Competencies.

  18. Receive proper supervision as per their Licensed Board.

  19. Additional duties as assigned.

In addition to the responsibilities listed in this job description, it is understood that each department/program may have specific requirements and/or regulations based on the level of care and treatments/services provided and/or the patient population receiving those treatments/services. These specific responsibilities will be reviewed with applicants/team members during the interview process and again at time of hire.

Qualifications

Education, Knowledge, Skills and Abilities Required:

  1. Master's degree in Social Work (MSW) or Master's degree in Counseling, or a related field.

  2. Minimum of two years of post-licensure work experience in the same field.

  3. Ability to perform psychosocial assessments and develop treatment plans.

  4. Knowledge of crisis intervention theory, its application in a variety of settings, as well as knowledge of disease management and disease process and differential diagnoses, and treatment techniques.

  5. Ability to work as part of a team, while building collaborative relationships.

  6. Must possess excellent follow through and attention to detail.

  7. Must possess excellent written and verbal communication skills.

  8. Proficient computer skills that may include but are not limited to Microsoft Office and/or Google Suite platforms.

Education, Knowledge, Skills and Abilities Preferred:

  1. Ability to provide guidance/mentorship to other team members.

Licenses and Certifications Required:

  1. Professional Counselor License or NJ Licensed Clinical Social Worker or Licensed Marriage and Family Therapist
