Bellefaire JCB helps more than 43,000 youth and their families yearly achieve resiliency, dignity and self-sufficiency through its more than 25 programs.

Position Summary:

We are looking for an Independent Living Case Manager to provide case management and community support program services to clients, their families, and/or significant others as needed. The Independent Living Case Manager transitions clients in community settings and works to provide supervision, assistance to daily living, case coordination, counseling, guidance and transportation. You will be a member of a treatment team and will be expected to meet the program’s productivity target for billable service each week. Services take place in the home and community with flexible work hours, some evenings may apply.

Responsibilities Include:

  • Accept assignment of cases from the supervisor to provide Community Support Program Services for clients, their families, and significant others as needed

  • Supervise a psycho educational group, assess each client to determine their proficiency in life skills and their ability to function in a group, and utilize group work skills to supervise a safe, growth-producing environment

  • Participate in the development of the client's ISP and crisis intervention work, where you provide assistance and support

  • Accepts assignments from the clinical supervisor and jointly develops tasks and interventions that implement treatment goals.

  • Recognize the significance of the primary family in the client’s life and demonstrate the skills to engage the client, primary family and other supportive adults in the treatment planning process.

Qualifications:

  • A minimum Bachelor’s Degree in Psychology, Social Work or related field.

  • A valid driver's license with approved driving record (less than 6 points) is required.

  • Must be able to ascend/descend stairs to visit client apartments.

  • Must be able to move about to take clients shopping.

  • Must be able to move 20 lbs.

  • Experience working with children, adolescents and their families.

  • Sensitivity in relating to persons of varying backgrounds and ideal candidate must demonstrate the ability to work with diverse groups of people possessing various strengths, aptitudes and abilities.
