Ellis Medicine Registered Nurse - Primary Care FHC in Schenectady, New York

Basic Function: The Registered Professional Nurse cares for patients by diagnosing and treating actual or potential health problems in the ambulatory setting without discrimination to age or gender through implementation of the nursing process. Coordinates care of other disciplines, delegates tasks to other nursing staff members, supervises care and evaluates results. Maintains competence in clinical practice and assists in the development of nursing practice standards through participation in professional committees and organizations. Requirements: Graduate of an approved Nursing program and is licensed to practice in New York State, BSN degree a plus. Previous nursing experience in an ambulatory setting preferred.

Responsibilities: • Assess, plan, direct and evaluate total nursing care as determined by the patient’s age related physical, psychological, and cultural needs in accordance with established standards, and policies. • Administers medications and intravenous solutions skillfully and correctly, and performs lab/diagnostic testing as ordered by provider and reports all critical lab values immediately. • Anticipates and prepares examination room with equipment and supplies required for the visit. Assists provider with examination and procedures per request. • Develop a NCP based on patient assessment. Attention given to developmental stages of the patient/family. Uses nursing practice standards and protocols, establishes realistic measurable goals and establishes time frames for goal achievement for the visit and over time of the relationship. • Identifies physiological, social, psychological and spiritual needs of patient/family. Identifies nursing interventions to meet goals. • Evaluates assessment data and establishes nursing diagnosis. • Prioritizes patient needs and selects diagnosis which can be addressed in the ambulatory setting. • Delegates patient care tasks to non-RN staff members. • Observes, interprets, reports and documents care given directly and by other nursing staff members. • Communicates the patient’s plan of care to all members of the health care team. Maintains communication through timely documentation in the medical record. Communicates with physicians and other providers on changes of patient condition and patient responses to medical therapy within a timely manner. • Provides patient education focusing on healthy development and wellness. • Provides patient education focusing on healthy development and wellness. • Assumes responsibility for education and own professional development and Maintains competency in nursing practice and keeps up with new knowledge and advances in the profession. • Incorporates nursing knowledge in nursing practice and utilizes time and resources available to maximize knowledge and skills. Salary Range: $30-$39.89/hour Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location.
