Housing Case ManagerResponsibilities of the Housing Case Manager is to provide services to adults who have a diagnosis mental illness, behavioral health issues, substance abuse, HIV/AIDs, and chronic homelessness. Specific duties included:- Case management services including benefit entitlements and case management.- Securing safe and affordable housing along with completing the necessary financial paperwork for a rental stipend.- Teaching the necessary skills to maintain stable housing.- Developing linkages to community services; and- Develop support plans for clients to utilize community resources when they need services or support.RequirementsSkills/Qualifications for the Housing Case Manager include: - College degree in human services and/or equivalent experience in the human service field especially in the populations diagnosed with mentally illness, behavioral health issues, substance abuse, and chronic homelessness.- 1 year case management experience.- Valid and insurable driver's license.- Both the ability to work independently and as a team member.- Excellent communication skills both verbal and written; and- The ability to be organized and have good time management skills.BenefitsSalary and Schedule of the Housing Case Manager:- 40 hours per week.
