Hospice of Southern Maine is seeking full time Transitional Care Nurse. At this time, this position is Monday through Friday; however, it includes the possibility of a weekend rotation expectation in the future.

As a key member of the Access Department RN team, the Transitional Care Nurse (TCN) is responsible for assuring that Hospice of Southern Maines programs are effectively represented and accessible to patients, families, referring physicians, and others with an interest in hospice. The TCN role is primarily focused on facilitating transitions of care for hospice-eligible patients in hospitals, ensuring timely access to hospice services according to agency mission and goals. As such, the TCN is a hospital-based field position, working closely with HSM Community Liaisons to establish and maintain daily relationships with hospital personnel. The TCN is additionally responsible for carrying out Access RN duties across care locations, including but not limited to pre-admission evaluations and informational visits, intake and referral coordination, admissions, other RN assessment visits as needed, and community outreach and education.
