Kaiser Permanente Case Manager RN, Cardio Vascular Care - On/Call Day in San Francisco, California
Job Summary:
Responsible for coordinating patient care throughout the cardiovascular care continuum and ensures the effective coordination and continuation of patient care from the pre-surgery to post-surgery treatments.
Essential Responsibilities:
Coordinates pre-surgery care services. Responsible for the initial intake and setting up the patient coordination process with external providers.
Determines availability of operating rooms and books cases accordingly. Completes surgical work ups for referrals. Coordinates transmission of clinical and benefit information to patients, families and outside agencies.
Coordinates the clinical management of post-surgery patients. Manages post discharge planning to ensure the continuing care of patient which include follow up appointments, educational programs and outside services for continuing care.
Provides patient education and consultation for patient and family members throughout treatment.
Basic Qualifications:
Experience
- Minimum three (3) years of patient care experience required.
Education
Bachelors degree in nursing or health related field, OR four (4) years of experience in a directly related field.
Graduate of an accredited school of nursing.
High School Diploma or General Education Development (GED) required.
License, Certification, Registration
- Registered Nurse License (California)

Preferred Qualifications:
Minimum six (6) months of previous case management experience preferred.
Minimum one (1) year of care coordination or cardiovascular patient care services experience preferred.
Experience in utilization management, discharge planning, or transfer coordination.
PHN preferred.
Cardiac nursing experience, Valve Care Coordination, Clinic Coordinator, scheduling experience.
