Sevita Licensed Vocational Nurse in San Dimas, California

NeuroRestorative, a member of the Sevita family, provides rehabilitation services for people of all ages with brain, spinal cord and medically complex injuries, illnesses and other challenges. In a variety of locations and community-based settings, we offer a range of programs, including vocational and therapy programs, day treatments, and specialized services for infants, children, adolescents, Military Service Members and Veterans.

Our NeuroRestorative facility located in San Dimas, CA, is a fully licensed JCAHO accredited Congregate Living Healthcare (CLHF) facility that specializes in brain, spinal cord and other life-altering injuries and medically-complex illnesses. We offer in-patient treatment in a residential home-like setting to individuals of all ages, which includes neurorehabilitation, respite, post-acute, and sub-acute care. We offer a variety of services including amputee care, complex wound care, G- and J- tube feedings, Occupational Therapy, Physical Therapy, Speech-Language Pathology, multiple IV antibiotic therapy, pulmonary care, social & recreational activities, and more.

Licensed Vocational Nurse

  • San Dimas, CA

  • San Dimas NeuroRestorative facility located on Puente Street

  • 15 bed facility

  • In-patient residential setting sub-acute physical rehab

  • Available positions:

  • Fulltime dayshift

  • Part-time dayshift & NOC shift

  • Per-diem dayshift & NOC shift

  • Days (7am-7pm) and NOC (7pm-7am)

  • Dayshift rates: $28-$31.50 base pay rate based on experience + OT after 8th hour = effective pay rate of $32.67-$36.75

  • NOC shift rates: $28-$31.50 + $2 NOC shift diff = $30-$33.50 + OT after 8th hour = effective pay rate of $35.58-$39.08

  • 12-hour shifts w/overtime after 8th hour (4 hrs of OT per shift)

  • DT after 12th hour

  • Low patient ratios

    The heart of what we do is support and serve people with brain injury.

  • Conduct assessments, implement health care plans, and respond to changes of condition

  • Communicate with the physician and external medical centers to ensure consistency of essential medical information

  • Monitor and ensure physicians orders are documented and carried out as prescribed

    Qualifications:

  • Current state LPN/LVN License

  • Current CPR certification

  • Current physical dated within past 1 year

  • Excellent assessment skills with the ability to exercise critical thinking in the clinical environment

  • Effective communication skills – ability to communicate with team members, medical professionals, those we serve, and their families
