Job Summary: As a Case Manager for the PSOP / Child Welfare Targeted Case Management (CW-TCM) Program you will conduct comprehensive assessments of families' strengths and needs, and, together, make decisions about what services or community resources are the best choices for success. This is a voluntary program that takes a family-centered approach to case management to work on areas of need identified by the family through a strength and relationship-based approach. The goals of this program are to help families to access resources, enhance the well-being of the family, and support family safety and stability. You work to coordinate services and other services designed to help children under 21 years old and the child's family gain access to needed social, health/mental health, habilitative, educational, vocational, recreational and related services including, but not limited to advocacy, transportation and legal services. All families from this program reside in Ramsey County. Qualifications and Requirements: Bachelor's degree in Social Work, Psychology, Sociology, or a closely related field from an accredited four-year college or university. A field other than social work, psychology, sociology or closely related field, plus one year of supervised experience in the delivery of social services to children as a social worker in a public or private social services agency. A valid driver's license, acceptable driving record, reliable transportation, valid insurance, and a Motor Vehicle Check. Strong organizational skills. Maintain records and reports as required by Ramsey County, DHS, and LSSMN, adhering to agency policies and administrative directives, and maintaining confidentiality of client information. Ability to be focused and attentive for an extended period of time. Able to develop, guide, and provide follow-up for client transition plans. Ability to promote a positive attitude regarding LSS and other programs to the public. Knowledgeable regarding community programs and referrals.
