COME WORK WITH US! Great opportunity for someone who has a passion for working with Intellectually Disabled clients. For immediate hire! $21/Hour. We are searching for a Part Time Case Manager based out of our Sacramento office.

WHO WE ARE: We've grown from one location in 2016 to 11 locations serving multiple regional centers/counties throughout California. We provide supportive living services in the privacy of our client's homes while protecting the rights of our clients and advocating for their needs. We are looking to hire people with a passion for providing services that meet and attempt to exceed expectations.

General information about the position:

Providing supervision of Direct Care staff and coverage coordination. Participate in training and supervising staff. Attentiveness of reportable events and submitting reports within required time frames. Serve as liaison with service coordinators, families, other agencies, and internal staff. Weekly review of client charts/records, and monitoring Daily Shift Notes. Supervision and evaluation of service delivery quality to clients. Coordinates and ensures staff and the program’s daily activities are completed efficiently. Must maintain a current working knowledge of Supported Living Services, and Federal and State laws, including familiarity with Title 17 regulations. This is NOT a remote/hybrid position. This job requires some occasional travel, to clients' residencies, and meetings at various locations within the area.

Quality Assurance and Compliance: Conduct regular quality assurance audits of client binders.

Responsibilities:

Client Home Binders and Office Binders: Oversee the organization and maintenance of client home binders and office binders, ensuring that all required documentation is present and up-to-date. Conduct regular audits of client binders to ensure compliance with individualized support plans, medication administration records, and other documentation requirements. Collaborate with direct support staff and case managers to address any issues or concerns identified during audits.

Qualifications:

Education:

Associate’s degree or equivalent may be substituted with work experience. Bachelor's degree preferred, but not required. 

Experience:

1 year of People Management experience.

1 year of experience in Case Management or:

2 years of experience providing services to individuals with Mental Health Challenges/Intellectual Disabilities.

Prior experience with general administrative tasks and documentation monitoring experience a plus. 

This has the potential for growth in a solid career field.
