Community Care Network is looking for an Advanced Practice Registered Nurse to join our team in Rutland, Vermont!

About the Role:

The Advanced Practice Registered works with a team of professional social workers, clinicians, case managers and others to ensure integrated care for adults experiencing serious mental illness. As a member of the client care team, the APRN will establish therapeutic relationships in order to facilitate client progress with treatment goals. To be effective, the APRN will have the demonstrated ability to balance independent clinical decision making with appropriate consultation from providers.

Qualifications:

Masters Degree in Nursing along with the appropriate Nurse Practitioner certification required.

Psych certification or candidate must be working towards psych certification.

Unrestricted, current and complete State of Vermont APRN license required (or ability to attain upon employment).

Experience working with adults with severe and persistent mental illness, substance use disorders, developmental disabilities, and co-occurring developmental disabilities and/or medical conditions required.

Demonstrated current knowledge of psychiatric medications.

Solid computer skills and familiarity with operating an Electronic Medical Record required.

Ability to demonstrate compassion, sensitivity, and reasoning skills.

Must have a valid drivers license, reliable transportation with personal automobile liability insurance coverage at or above the levels mandated by the State of Vermont.

*Successful completion of a criminal history background check after hire.
