Catholic Health Services RN Oncology Patient Navigator in Rockville Centre, New York

The oncology nurse navigator is a professional registered nurse with oncology specific clinical knowledge who offers individualized assistance to patients, families, and caregivers to help overcome healthcare system barriers. Using the nursing process, and ONN provides education and resources to facilitate informed decision making and timely access to quality health and psychosocial care throughout all phases of the cancer continuum. Job Details Position Requirements and Qualifications Performance Responsibilities: ONN demonstrates professionalism within the work place and the community, through effective teamwork. Promotes lifelong learning and EBP to improve the care of patients with past, current, or potential diagnosis of cancer. Obtains or develops oncology related education materials for patients, staff and community members as needed. Participates in the tracking of metrics and patient outcomes, in collaboration with the quality and administration, Promotes a patient and family centered care environment for ethical decision making. Acts as an advocate for cancer patient. Build partnerships with local agencies and groups to educated and provide assistance to cancer patients. Establish a support group, and provide education to patient, families and caregivers in collaboration with oncology social worker. Assisting patients with schedules, appointments and treatment courses. Ensures that appointments/procedures for treatments are scheduled. Demonstrates knowledge of clinical guidelines. Attend and participate in staff meetings, conferences and tumor board conferences. Educates patients on treatment side effects and what to experience prior to start of treatment. Completes a patient navigation assessment on each new oncology patient for the physician practice. Ensures documentation of patient encounters and services. Assists with identifying candidates for genetic counselling. Participates in cancer committee and assisting with COC Cancer Designation. Partners with leadership to execute navigation goals for program development. Support a smooth transition of patients from active treatment into survivorship or end of life care. Experience: Minimum of 2 years experience as an RN required. Minimum of 3 years oncology clinical experience required. Licensure/Certifications: Current NYS license as a Registered Nurse. BLS Certification required. OCN Preferred, expected within first year of role. Education: BSN preferred Skills: Bilingual (Spanish) preferred. The ONN works collaboratively with the healthcare team using clinical expertise to educate, and connect patients to healthcare team, assisting patient and families to overcome healthcare system barriers, and facilitate decision making. Knowledge of community resources, and standards of care, ability to interact with community groups, and possess excellent written and verbal communication skills. Physical Demands: Position is moderate and will require sitting for long periods of time interspersed with walking to and from various locations. Excellent verbal and listening skills required for communicating clearly and effectively with others. Ability to bend, stretch, reach, handle/finger delicate items required. Ability to carry files up to 5 lbs. Reasonable accommodation will be made to enable individuals with disabilities to perform the essential physical demands. Salary Range USD $53.08 - USD $60.40 /Hr. This range serves as a good faith estimate and actual pay will encompass a number of factors, including a candidate’s qualifications, skills, competencies and experience. The salary range or rate listed does not include any bonuses/incentive, differential pay or other forms of compensation that may be applicable to this job and it does not include the value of benefits.
