JOB RESPONSIBILITIES

  1. Actively support and further the Mission of The Arc, To empower people with disabilities to believe in their own unique abilities and achieve their full potential by providing quality, innovative services that focus on inclusion, advocacy, independence, employment, meaningful community life, and personal happiness.

``` 2. Work in accordance with Illinois Administrative Rule 116 and in conjunction with other personnel of The Arc to provide supportive assistance to individuals served by:

``` 1. Administering prescribed medication and/or treatments and maintaining necessary documentation in accordance with Illinois Administrative Rule 116 and policies and procedures of The Arc. 2. Delivering instruction and/or health care on an individual and/or small group basis to designated individuals and in accordance with policies and procedures of The Arc. 3. Assessing individuals and maintaining necessary data documentation in accordance with prescribed guidelines. 4. Participating in individual staffings and IDTs when appropriate.

``` 3. Provide supervision and ongoing assistance in personal and social development of designated individuals served by The Arc. 4. Participate in conferences, seminars, meetings and in-service training sessions as requested. 5. Responsible for making recommendations to supervisor to ensure the health and safety of individuals served by The Arc. 6. Attend team meetings and training sessions when advised. 7. To perform all tasks and duties as assigned by supervisor.

QUALIFICATIONS

LPN with graduation from an accredited nursing school.

LPN licensed in the State of , current and in good standing.

Valid drivers license in good standing and vehicle with appropriate auto insurance required. Must be approved by Agencys Auto insurance carrier for driving agency vehicles.

Demonstrated strength and flexibility to assist in lifting individuals or provide other individual related care.
