Summary
The Supportive Care Social Worker is responsible for the delivery of skilled assessment and intervention for identified CompassionNet patients (families with a child with a potentially life-threatening illness). This position provides and coordinates psychosocial support to enrolled families and offers assistance with psychosocial, financial, spiritual stressors that impact child/family's quality of life. The Supportive Care Social Worker collaborates with all members of the interdisciplinary care team and documents all care activities.

SCHEDULE: Day

ATTRIBUTES

  • LMSW or LCSW coupled with a minimum of 3 years pediatric and/or palliative care experience.
  • Demonstrated sustained high performance in role as evidenced by 2 or more consecutive performance reviews exceeding expectations.
  • NYS Drivers license and person vehicle required
  • Demonstrates strong communication (both written and verbal), interpersonal, organizational, and time management skills.
  • Basic computer skills are necessary, including but not limited to the ability to navigate on a personal computer within a Windows based operating environment; the ability to access and create basic e-mail messages; and the ability to open, edit, and save basic electronic documents if necessary.
RESPONSIBILITIES
  • Community Outreach. Provides community outreach/education regarding program and eligibility. Actively involved and is responsible for training new staff.
  • Assessment. Performs thorough psychosocial assessment from a family-centered care perspective. Provides anticipatory guidance and support for coping with the disease process from a family-centered perspective. Performs comprehensive and ongoing assessment of child and family coping, quality of life indicators, and family system integrity. Collaborates with family and interdisciplinary team to identify options, goals and priorities for care, advanced care planning.
  • Documentation. Documents all communication/interventions with all program participants and potential participants. Provides emotional/psychosocial support and referrals/financial support for community resources as assessed to facilitate effective coping for family members. Responsible for approval and management of contracted services for patients/families on caseload.

PHYSICAL REQUIREMENTS: S - Sedentary Work - Exerting up to 10 pounds of force occasionally Sedentary work involves sitting most of the time, but may involve walking or standing for brief periods of time. Jobs are sedentary if walking and standing are required only occasionally and all other sedentary criteria are met.

PAY RANGE: $55,500.00 - $75,500.00

The listed base pay range is a good faith representation of current potential base pay for successful applicants. It may be modified in the future. Pay is determined by factors including experience, relevant qualifications, specialty, internal equity, location, and contracts.
