University of Rochester Supervisor, Social Worker in Rochester, New York

GENERAL PURPOSE:

The senior social worker, in collaboration with the interdisciplinary care team, provides social work clinical services, including discharge planning, to individuals and families of all ages and is responsible for the recruitment, training, scheduling and supervision of social work staff. The senior social worker, in conjunction with the Associate Director of Social Work, ensures optimal psychosocial care to patients seen on the inpatient units.

SPECIFIC RESPONSIBILITIES:

  • Provides social work interventions to patients and their families including emotional support in crisis situations, information about and referral to community resources, facilitation of patient transfer to other care facilities and other services relevant to the patient's discharge from the hospital; Performs psychosocial assessments of patients identified through referral and/or risk assessment

  • Responsible for recruitment, training, and supervision for social work staff; provides onsite supervision opportunities.

  • Conducts annual performance evaluations and competency evaluations on assigned social work staff, and monitors their professional practice;

  • Participates in after hours on call schedule

  • Ensures that Hospital and Social Work Division policies and procedures are implemented and adhered to by the assigned social work staff with regard to professional practice, patient care and discharge planning

  • Collaborates with physicians, nursing and Community Health Nursing staff in developing an interdisciplinary care and/or discharge plan for the patient

  • Provides appropriate assessment and intervention services for children and adults suspected of being victims of abuse, violence, neglect and/or domestic violence;

REQUIREMENTS:

  • The senior social worker must have an MSW from an accredited social work graduate program and must obtain New York State license within eighteen months of employment required.

  • Five years clinical experience in a medical setting and supervisory experience preferred.

  • The social worker should be familiar with medical social work and community resources preferred.

Pay Range

Pay Range: $64,480 - $83,824 Annually

The referenced pay range represents the minimum and maximum compensation for this job. Individual annual salaries/hourly rates will be set within the job’s compensation range, and will be determined by considering factors including, but not limited to, market data, education, experience, qualifications, expertise of the individual, and internal equity considerations.
