Responsibilities

General Purpose:

The Senior Social Worker has administrative and supervisory responsibilities (40%) supporting the social workers within the Division of Collaborative Care and Wellness and clinical responsibilities (60%) as an integral member of the interdisciplinary primary care team at Highland Family Medicine. They work closely and in collaboration with the Program Director in the management of clinic operations of the services and to establish and maintain liaison with the social work discipline. They provide screening, psychosocial assessment, psychiatric diagnosis and brief, evidence based treatment for patients and families being served at HFM and enrolled in the integrated OMH-regulated clinic. In this role, the Sr. social worker routinely partners with others in the healthcare team (nursing, care management, social work, clinicians) to identify high risk patients with behavioral health and medical comorbidities, contributes to a unified person-centered treatment plan with the goal of improving the patient’s overall health and well-being, and facilitates access to other specialty psychiatric and substance use care as needed. The position will require formal and informal collaboration with all clinical team members, in –person and through the electronic health record.

Responsibilities:

Recruits, supervises and evaluates the performance of social workers in the division of CCW, including quarterly audits of electronic medical records. May also collaborate around the recruitment, supervision, and evaluation of other discipline clinicians, as appropriate.

Provide discipline specific administrative supervision and support to social work staff in the Collaborative Care Division including those outside of HFM.

Provides and participates in clinician/staff training and education in integrated behavioral health primary care model.

Performs psychosocial assessment and diagnostic formulations with patients and works with all clinical team members to mutually enhance understanding of the patient/family.

Ensures the development, implementation and monitoring of comprehensive and individualized biopsychosocial treatment plans.

Offers longitudinal interventions with patients including functional and strength-based assessment and diagnosis; psychoeducation for patients and their support systems; medication adherence; motivational interviewing; and brief problem solving interventions aimed at promoting self-efficacy, enhanced decision-making, and behavior change.

Initiates follow up to ascertain how patients are doing and to determine if changes in the treatment approach are indicated.

Helps to oversee the quality assurance of clinical services, including working with social work staff to ensure that documentation meets SMH, OMH, and other regulatory agency standards.

In consultation with the HFM Behavioral Health Clinic Director, monitors productivity to meet Departmental and Divisional benchmarks. Participates in the development and/or execution of outcome measurement initiatives in the service area.

Ensure that Hospital, Department, and Social Work Division policies and procedures are implemented with regard to patient care, discharge planning, and professional practice.

Assists in tracking and monitoring, including patient’s adherence to treatment plans, reported outcomes tools, and status after referral to specialty mental health.

Recommends and facilitates which patients should be referred to a specialty mental health facility, wellness programs; and or other community agencies.

Documents all assessment and treatment plans in patient’s electronic medical record in compliance with Division, Department and Hospital policy

Provides back up mental health service coverage to other HFM integrated therapists, as needed. Provides child/adult protective consultation to clinicians and other HFM team members as needed. Provides crisis intervention and support, during working hour and participates in on-call activities at HFM, as indicated.

Provides recommendations and decision support to the primary care team concerning the patient’s treatment plan and goals.

Functions collaboratively and consultatively as a member of the interdisciplinary team.

Attends Social Work Division, Psych dept, Program Staff and Team Meetings, as indicated.

Participates in the routine committee process of the Social Work Division as appropriate, and complies with Social Work Division quality assurance and staff development activities.

Participates in activities of the Social Work Division including timely completion of time reporting and other Division forms; Hospital and Division mandatory in-service training programs and UHS reviews.

Subscribes to the UR Medicine goal of creating a culturally competent environment by treating patient, families, trainees, and co-workers in a sensitive manner with appropriate attention to cultural differences. Participates in available cultural competency events and training appropriate to job duties.

The Senior social worker’s professional performance is supervised by a Social Work Division supervisor who is licensed in the State of New York.

The Senior social worker is accountable administratively and clinically to his/ her social work supervisor for provision of social work services, and adherence to Social work policies, procedures and standards of professional conduct. The social worker’s job performance must conform to the expectations set forth in the position description.

The Senior social worker is accountable to the UR site Medical Director and the Director of Collaborative Care and Wellness Division for daily clinical and operational matters and for providing professional clinical services in accordance with the policies and standards established by the primary care /office management team and the Dept. of Psychiatry

Qualifications:

  • The social worker that occupies this position has an MSW from an accredited social work graduate program.

  • Knowledge about and ability to provide behavioral health assessments/evaluations and evidenced based interventions;

  • Commitment to and routine demonstration of biopsychosocial approach to care of individuals and families;

  • Dedication to delivering a team-based integrated model of care.

  • Demonstrates cultural humility and anti-racism values and works toward ensuring all patients feel welcome.

  • They must be licensed by the State of New York (LCSW or LCSW-R)

Preferred:

  • Training and clinical experience in integrated behavioral health and collaboration preferred. 1-2 years supervisory experience

Pay Range

Pay Range: $ 64,480 - $ 83,824 Annually

The referenced pay range represents the minimum and maximum compensation for this job. Individual annual salaries/hourly rates will be set within the job’s compensation range, and will be determined by considering factors including, but not limited to, market data, education, experience, qualifications, expertise of the individual, and internal equity considerations.
