University of Rochester Social Worker in Rochester, New York

Responsibilities

General Purpose:

The social worker provides consultation and clinical social work services to URMC Primary Care Network patients and their families pursuant to the PCN Patient Centered Medical Home. Social work services are provided in a centralized model across primary care offices, and is not embedded in any one office, nor is the position remote from home. The social worker collaborates with the health care team to assist patients identified as needing social work intervention. The social worker provides consultation to health care providers regarding patient care needs/community resources and documents all information in the patient’s electronic medical record. The social worker may meet with the patient in the primary care setting to offer further assessment and engagement. May make home visits as indicated.

Responsibilities:

  • Assesses needs of patients and families referred by care managers and providers. These patients include, but are not limited to; elderly, children or disabled persons in need of resource assistance or protection; persons experiencing emotional crises; and patients with new, chronic or terminal diagnoses. Provides social work services to address needs including complex SDOH issues across a rural region of primary care practices. Other services include crisis intervention, transitional, long term care and respite care planning, end of life support, adjustment to diagnosis supportive counseling, information and referral, resource mobilization and advocacy as indicated. Coordinates referrals to and acts as a liaison to community agencies.

  • Assists in the identification and management of possible child maltreatment, IPV, elder abuse and abuse of other vulnerable patients per Social Work Division policies.

  • Documents all social work activities in the patients' medical record in accordance with the relevant Social Work Division policy and procedure.

  • Participates in SWD and PCN meetings and activities including but not limited to timely completion of statistical reports/forms, time reporting and other PCN or Division forms, mandatory in-service training, and UHS reviews.

  • Participates in social work supervision/consultation about his/her work and professional development plan. Participates in relevant professional development trainings.

  • Provides coverage services as requested during the absence of co-workers

Other duties as assigned

Qualifications:

  • Masters in Social Work from an accredited program required.

  • Experience in a medical or clinical setting required.

  • Strong interdisciplinary, collaborative skills.

  • Social Work licensure required within 18 months required.

Pay Range

Pay Range: $ 57,595 - $ 80,600 Annually

The referenced pay range represents the minimum and maximum compensation for this job. Individual annual salaries/hourly rates will be set within the job’s compensation range, and will be determined by considering factors including, but not limited to, market data, education, experience, qualifications, expertise of the individual, and internal equity considerations.
