UR Medicine Home Care Medical Social Worker - Hospice in Rochester, New York

Identifies, assesses and manages the psychosocial needs of patients/families relating to illness and end of life care. Develops and implements appropriate plans as part of an interdisciplinary team. Provides counseling, education, advocacy and consultation.

RESPONSIBILITIES:

  • Continually assesses the patient/family psychosocial status including strengths, coping mechanisms, personal goals, significant relationships, home environment and financial status.

  • Participates in the Hospice patient assessment process, providing patient and family member with support and information on the Hospice program and disease process.

  • Provides on-going social work services to patients/families that conform to regulatory and professional standards, NASW Code of Ethics and VNS mission, vision and values. These services include counseling, advocacy, education, and referral to appropriate community resources.

  • Establishes and maintains an on-going relationship with the patient/family/significant others.

  • Assists patients/families through this relationship to adjust to any medical, social emotional problems which may arise.

  • Participates in the interdisciplinary team providing social work input to address the psychosocial aspects of patient care in the development of the patient’s treatment plan.

  • Provides bereavement assessment and follow up to families and refers to bereavement coordinator as appropriate.

  • Participates in Hospice in-service presentations to community groups and referral sources.

  • Maintains timely documentation of events and all required paperwork per agency policy.

  • Acts as preceptor for agency social workers and social work interns.

  • Maintains productivity standards/requirements.

  • Responsible for the Key Performance Indicators (KPI’s) established by their manager.

  • Performs other related duties as requested or assigned.

    QUALIFICATIONS:

  1. Bachelor’s degree in Social Work required. Master’s degree in Social Work (MSW) preferred.

  2. Minimum of two years experience working in the social work or related field.

  3. Working knowledge of community resources.

  4. Effective interviewing, oral and written communications skills.

  5. Excellent interpersonal skills.

  6. Ability to function independently.

Salary Range: $18.33 - $38.90
