Description

Under supervision, provides medication management and offers general health and wellness support. Works collaboratively with other providers in the residence to facilitate client wellness. Maintains confidentiality and ethical standards of relevant governing bodies. Effectively supports ongoing and collaborative communication amongst all program staff and is responsible for ensuring a trauma informed care environment.

Essential Duties and Responsibilities

  • Provides medication management within the residential setting

  • Provides medication administration to clients in the residential environment in accordance with authorized prescriptions

  • Documents the medication administration process in each client’s Medication Administration Record

  • Procures and logs in medications received for each client within the facility

  • Tracks client compliance with required medications, identifying any concerns to other Prescribers and other medical staff

  • Reports medication compliance to the clinical team

  • Coordinates directly with Residence Manager to ensure shift coverage for unapproved or unplanned absences.

  • Provides Day to Day support of client wellness within the setting

  • Observes and identifies potential client medical or medication concerns

  • Facilities identification of medical concerns to appropriate medical or psychiatric staff or prescribers

  • Assures identified medical concerns are addressed

  • Communicates and collaborates with appropriate Agency staff, Medical care providers, and related community organizations to assure coordination of client care and services.

  • Completes required paperwork related to communication or services provided

  • Coordinates with appropriate residential staff to facilitate transportation to necessary medical appointments

  • Promotes health and wellness within the residential facility

  • Supports the development of client wellness education and activities

  • Assists Medical Staff and Prescribers

  • Provides Support with vital signs, and other assistance to providers as needed

  • Collaborates with appropriate residential staff on client discharge plans

  • Participates in staff meetings and medical/clinical supervision as directed

  • Participates in relevant Agency meetings and/or trainings

  • Actively pursues development of professional competencies related to job role performance through reading, supervision, in-service training and attendance at conferences and workshops.

  • Awareness and active support of the Agency's Mission, Vision, Values and Strategic Plan. Including, but not limited to, supporting the Agency's Core Competency initiatives of being Collaborative and Community Focused, Innovative and Entrepreneurial, and Culturally Competent and Diverse

  • Other duties as assigned

Qualifications

Education: Minimum of a High School Diploma or GED and accredited LPN diploma required.

Credentials: Valid Licensed Practical Nurse required

Experience: Two to three years of experience related to nursing services to individuals with mental health, alcoholism, chemical dependency, substance use disorder related services preferred. Bi-lingual in Spanish preferred.
