CDS Life Transitions Job Developer in Rochester, New York

OVERVIEW:

Responsible for all activities relating to job development and placement of individuals in competitive employment.

RESPONSIBILITIES:

  • Establish and maintain contacts with businesses within the community to identify potential employer and/or potential providers of community-based assessment/volunteer sites.

  • Establish and maintain a relationship with ACCES-VR; maintaining contact with Vocational Rehabilitation Counselors and ACCES-VR liaison, as well as attending ACCES-VR meetings as scheduled.

  • Process referrals from ACCES-VR.

  • Prepare and submit reports to ACCES-VR by due dates established by ACCES-VR and CDS Monarch.

  • Complete and maintain all trainings that are required by ACCES-VR.

  • Assist individuals in making contacts with employers and preparing for interviews.

  • Perform job analyses; match job requirements to individuals’ skills and interests; make job placements.

  • Work with employment staff to design on-site training plans.

  • Establish and maintain contacts with transition groups and school systems to identify potential individuals for transitional services.

  • Provide job coaching support to individuals upon obtaining a job.

  • Coordinate work study placements to include teacher/student/parent-planning meetings, setup and monitoring of work/studies, coordination of wrap-up meetings.

  • Conduct pre-placement workshops and training to transitioning students.

  • Ability to capture all services and attempts to provide services through timely documentation.

  • Attend Life Plan meetings for those on caseload, as well as cover meetings as needed.

  • Maintain compliance for each individual regarding Life Plans, Staff Action Plans, Addendums, etc.

  • Maintain, update, and back fill individual cases.

  • Ensure that services provided to individual are appropriate and documented.

  • Perform all other duties relevant to this position as assigned by supervisor or designee

ESSENTIAL ACADEMIC AND EXPERIENCE REQUIREMENTS:

  • Must be at least 18 years of age.

  • Bachelor Degree in Human Services field or equivalent experience required

  • Previous experience providing ACCES-VR services preferred

  • Experience with business development

  • Reliable transportation (NYS Driver’s License required).

  • Ability to work independently and manage time effectively to meet department expectations

  • Ability to communicate effectively, both orally and in writing.

  • Ability to lift at least 50 lbs. dead weight.

  • Maintain all required certifications/training by State regulations and CDS Monarch policy.

  • All experience and education requirements, except when required by federal, state, or local laws or requirements, may be waived at the discretion of management with the approval of the Program Vice President in collaboration with Human Resources.

CORPORATE QUALIFICATIONS:

  • Adhere to all CDS Monarch policies and procedures.

  • Adhere to the Agency Mission, Vision, Shared Values, and Customer Service Standards.

  • Attend mandatory Education and Training modules as scheduled, obtains, and maintains required certifications.

  • Act as a professional representative of CDS Monarch in regards to appearance, behavior, temperament, communication, language, and dress.

  • Perform all other duties relevant to the position as assigned by supervisor.

The listed salary range represents the organization's good faith and reasonable estimate of the range of possible compensation at the time of posting.
