About Us:East House is dedicated to providing comprehensive care management and support services to individuals in recovery from mental illness, chemical dependency, and physical illness. We are currently seeking a passionate and skilled Care Manager to join our team and make a positive impact on the lives of our clients.

Position Overview:As a Health Home Care Manager, you will play a pivotal role in partnering with clients to provide comprehensive care management, health promotion, transitional care, patient and family support, and referrals to community and social support services. You will use health information technology to link services and help individuals improve their health, live self-directed lives, and reach their full potential.

Key Responsibilities:

  • Services That Address Client Goals:

  • Conduct comprehensive health assessments to identify medical, mental health, chemical dependency, and social service needs.

  • Partner with clients to develop individualized service plans based on client-identified goals and objectives.

  • Manage caseload to coordinate client services in an extensive community network.

  • Interact independently with clients in their homes and in community settings.

  • Develop and maintain positive relationships with various stakeholders.

  • Provide continuity of care and health promotion.

  • Participate in regular case reviews and maintain knowledge of best practices.

  • Documentation Standards Met:

  • Maintain documentation to the standards defined by funders and East House policies.

  • Monitor and report on client outcomes as defined by funders and East House policies.

Results and Measures:

  • Comprehensive care management, care coordination, health promotion, transitional care, patient and family support, and referral to community and social support services.

  • Compliance with documentation standards and achievement of required metrics.

  • Frequency of client contact aligned with client needs.

  • Professional development and training.

  • Client, family, and provider satisfaction with assistance provided.

Physical/Mental Demands:

  • Sitting, handling objects, paperwork, computer work, and verbal/written communication.

  • Occasional lifting up to 30 lbs., walking, standing, and reaching.

  • Finger dexterity, hearing, and visual requirements.

  • Ability to read and interpret policies, procedures, and various information.

  • Ability to maintain emotional control under stressful situations.

  • Ability to travel and access client homes, which may or may not be handicap accessible.

Working Conditions:

  • Traditional office or household environment.

  • Pressure due to time demands.

  • Occasional travel to other East House facilities.

  • Availability to work on weekends and some evenings.

  • Frequent local travel and transportation of clients.

Requirements

Care Manager Job Levels:

  • Care Manager:

  • Bachelor's Degree + minimum 1 year experience in Health Homes Care Management

Licensure/Certifications:

  • NYS motor vehicle license, safe driving record, and availability of a personal vehicle for work.

Other Skills/Knowledge/Experience:

  • Demonstrated knowledge of counseling culturally diverse individuals with psychiatric and/or chemical dependency problems.

  • Skills in counseling, crisis assessment, intervention, and goal planning.

  • Effective communication and ability to work cooperatively with clients, staff, and community representatives.

  • Computer literacy, including basic internet and Microsoft Word skills.

  • Experience using multiple database programs.

Salary Description

$19.45-24
