POSITION SUMMARY:

The Hospice Registered Nurse functions as the primary nurse for hospice patients and families in the home setting. She/he is a member of the hospice interdisciplinary team. The Registered Nurse: a) sets nursing goals and plans, which demonstrate a holistic approach to the patients/families; b) mobilizes resources to meet the individualized needs of patients/families; c) assumes leadership responsibilities for their individual caseload and d) demonstrates competency of clinical skills.

Essential Functions

Initiates home care assessments on assigned patients within 48 hours of admission to hospice.

Initiates the patient’s plan of care I accordance with nursing needs specified in the approved plan of care

Assesses patient/family needs on initial visit and completes the appropriate documentation, including but not limited to the Initial Nursing Assessment, Pain Assessment and Medication Profile.

Develops and updates the nursing care plans per hospice policy

Re-evaluates patient/family with each visit at least weekly Delivers skilled nursing services as indicated.

Assesses patient/family needs for counseling and teaching of patient care skills; facilitates learning skills at home or on the unit if appropriate

Communicates relevant information to the Clinical Manager and other hospice team members as appropriate.

Supervises home health aides per hospice policy. C

Conducts in-home supervisory visits of home health aides bi-weekly and provides written HHA Care Plan prior to the home health aides’ initial visit to the home.

Demonstrates knowledge and practice of Infection Control and Safety Policy and Procedure.

Coordinates acquisition of medical supplies and equipment as appropriate for patient/family.

Participates in on –call rotation and responds appropriate to the family/patient requests or needs.

JOB REQUIREMENTS:

  1. Graduate of an accredited school of nursing. BSN Degree preferred.

  2. Current licensure to the practice as a Registered Nurse in the Commonwealth of Virginia.

  3. Current CPR certification

  4. Valid Driver's License and an acceptable driving record

  5. Competent Registered Nurse Possesses: a) broad-based knowledge of clinical, psychosocial and patho-physiological theories relating to nursing diagnosis and treatment; b) skills and ability to perform the range of nursing procedures and techniques found in general nursing practice and the ability to translate these skills to the home setting; c) demonstrated effective teaching skills. This level of knowledge, skills and ability may generally be acquired through 2 years current medical-surgical staff nurse experience in an acute care facility, or during 12 to 18 months acute care plus 6 to 9 months current experience in a home health related setting. Oncology experience preferred.
