Summary:

The Clinical Coordinator will ensure the continuity of care for the Med-surg/Acute Rehab unit patients. The Coordinator is responsible for the provision of care to the patients through management of a team who demonstrate excellent critical thinking skills, clinical judgment skills, and communication with all health care providers. The role will be responsible for providing clinical expertise in the provision of patient care and will collaborate with the health care team to implement Quality initiatives, performance improvement projects, and maintain regulatory compliance for the service line. The coordinator will assist in recruitment and retention for the night shift staff, nurses, pcts and unit secretaries.

Responsibilities:

  1. Works with the health care team to develop a plan of care with the patient, family/caregivers, and providers to insure safe, quality care with optimal outcomes.

  2. Will monitor adherence to care plans, evaluate effectiveness, monitor patient progress and provide ongoing communication to the health care team.

  3. Promote the patient’s ability for self-management through, patient and care giver education and shared decision-making through patient advocacy.

  4. Develops and implements policies and procedures that guide and support the provision of care for the patients.

  5. Provide input to the operations and capital budget.

  6. Assist in the development of department goals and quality indicators including, data collection and dissemination of data to appropriate stakeholders.

  7. Works collaboratively with Director and nursing staff through daily management of staffing as well as 6-week staffing schedule.

  8. Provide ongoing staff education, to include, but not limited to annual education, regulatory requirements, and patient safety drills.

  9. Provides effective leadership and continuous improvement of department activities either personally or through appropriate delegation.

  10. Maintain contemporary professional knowledge and education.

  11. Demonstrates regular, reliable and predictable attendance.

  12. Performs other duties as required.

Education Skills Experience

Bachelor of Science in Nursing (BSN) preferred.

BLS, ACLS
