Mindoula Health Licensed Clinical Program Director, Population Health Management in Remote, Louisiana

Mindoula is a next generation population health management company that identifies, engages, and serves populations with complex behavioral health, medical, and social challenges across the continuum of care. By using technology to “scale the human connection,” Mindoula helps health plans, health systems, hospitals, and provider groups extend their reach and achieve their value-based service delivery goals. At Mindoula, we address the full range of behavioral health challenges. We deploy tech-enabled teams of case managers, care managers, community health workers, peer support specialists, therapists, and psychiatrists to provide 24/7 support to even the most complex and underserved behavioral health populations.

The Program Director will build best in class teams to deliver a successful Population Health Management Program (PHMP) across multiple partners/locations for all stakeholders - individual members, health plan partners, and Mindoula. They will be expected to attract, train, and motivate a talented team to implement and deliver Mindoula's PHMP and the expected outcomes. Implementation and delivery includes executing on all contractual, legal, and regulatory requirements, as well as delivering on partner and company clinical and business goals and objectives. The Program Director also will provide clinical oversight to the team, to ensure that all interventions, interactions, and services are delivered to meet and exceed quality expectations.

What you'll do...

  • Lead the Population Health Management Program (PHMP), expanding the program and delivering on all financial and clinical goals.

  • Understand contractual obligations to the health care plan partner, in full, and ensure that all are met successfully at all stages of program implementation and delivery.

  • For new programs, create a plan to implement the program, in partnership with the GM of Specialty Population Health Management and the implementation team. Ensure it is scalable and sustainable, and then deliver. Solve problems as needed throughout the implementation to ensure all issues are resolved, goals are met and health plan partners are satisfied.

  • Deliver against monthly and quarterly enrollment objectives through SMART metrics and KPI's, implement improvements as needed to meet targets.

  • Elevate team performance by providing comprehensive coaching and mentorship and fostering a culture of excellence in service delivery with the goal of reducing avoidable utilization, lowering total cost of care, and promoting members over health and wellness.

  • Ensure all team members know how to perform the core functions of their job, including onboarding members, delivering interventions, and utilizing the internal system to monitor their assigned members to maximize engagement and deliver outcomes.

  • Collect and synthesize data that highlights member outcomes to support team training, improve high-quality engagement, deepen internal understanding across markets, strengthen relationships with our partners, and enable us to continually drive toward results.

  • Analyze team aggregate and individual activities, inputs and outcomes to develop and execute on continuous improvement plans to improve the quality of member interactions and improve outcomes, including member experience/satisfaction.

  • Partner with People Operations colleagues to hire, coach and mentor an exceptional team and to drive success in training, partnering with colleagues to close any gaps, locally and nationally, driving toward excellence.

  • Manage to and report out on operational budgets for the program and partner with the leadership team to ensure delivery within approved limits.

  • Identify Member's behavioral health diagnoses and ensure that the Member's service plan is comprehensive and appropriate for the Member's needs.

  • Partner with the billing team to review and approve claims, ensuring they accurately reflect services provided including any additional information as needed.

  • Ensure strict compliance to all local, state, federal laws, and health plan rules and regulations. Seek training opportunities for the team around compliance. Lead internal chart review for partner audits. Participate in a rotating on-call schedule to support our members.

What you'll need...

  • A Qualified Mental Health Professional (QMHP) holding one of the following licenses or equivalent:

  • Licensed Clinical Social Worker (LCSW)

  • Clinical Professional Counselor (CPC)

  • Licensed Marriage and Family Therapist (LMFT)

  • Licensed Psychologist (PhD or PsyD)

  • 10+ years of relevant population health experience.

  • Strong leadership experience managing a team

  • Experience leading, building and implementing programs nationally with strong outcomes.

  • Bachelor's degree in behavioral health, social work or health care management from an accredited university required.

  • Masters in public health, health care management or similar program preferred.

  • Management and compliance: Ensures care is delivered according to applicable laws, regulations, policies and procedures, supporting the organization's integrity efforts by acting in an ethical and appropriate manner and that the team is operating at a level of excellence befitting our brand. Proven ability to train, develop, and coach team members, fostering a culture of accountability and continuous improvement.

  • Relationship Management: develop, maintain, and strengthen partnerships with others inside or outside the organization. Able to work cooperatively and effectively with others to set goals, resolve problems, and make decisions that enhance organizational effectiveness.

  • Problem Solving: Identify needs and take independent action to implement change when and where it is needed, focusing on results. Able to plan, organize, manage time, and prioritize multiple tasks and assignments. Use effective follow through.

  • Communication: Excellent listening, verbal and written communication skills. Clearly and accurately, relaying information to staff, business partners, stakeholders, and clients.

  • Member and Crisis Management: Able to provide case/care management in routine and crisis situations.
