Sutter Health Physician Assistant Cardiology Redwood City in Redwood City, California

Position Overview:

Provide care management services to patients with chronic conditions, enrolled in the Chronic Care program, using the electronic health record system and personal health record system tools for management. Conduct patient education, group visits, and individual patient consults for patients with Depression, Diabetes, Hypertension and other cardiovascular risk factors. Participate in the development and revision of modules to expand the range of disease covered by the system. Provide clinical oversight to our Health Coaches within the Alameda division.

Perform protocol-based care management of patients under the supervision of the primary care provider. Participate in the development and operationalization of the care management program. In addition function as a team provider evaluating, treating, diagnosing patients same day sick and well appointments as well as performing Medicare wellness exams.

Candidate should have strong interest in chronic disease management, and be interested in implementing future programs in other chronic disease areas. Candidate should demonstrate strong clinical skills and have the ability to work independently to evaluate and treat sick and well visits.



  • Current license to practice in the state of California as an PA-C

  • BCLS

  • DEA and Furnishing License

  • National Certification


  • 2 years related clinical experience

  • Experience with managing patients with chronic diseases
