UNC Health Care Patient Navigator Bariatrics in Raleigh, North Carolina

Description

Summary:

Coordinates all aspect of the pre-surgery process to support bariatrics patients including scheduling and teaching educational classes, managing all insurance requirements, coordinating with surgeons, and tracking patient progress.

Responsibilities:

  • Completes a comprehensive barrier assessment in four domains: health promotion (i.e. access to medical care, food insecurity, tobacco cessation and exercise), logistics (i.e. transportation, lodging, parking), financial (i.e. completion of internal and external assistance applications, foundation monies), and emotional and social support (i.e. coping resources, referrals, advance directives, and community engagement).

  • Navigates patients, families and caregivers through the pre-surgery process providing resources, education, advocacy, referral and support.

  • Coordinates all insurance requirements to ensure proper documentation is submitted to the insurance authorization team.

  • Coordinates and facilitates 1:1 and group educational classes for patients throughout the pre-surgery process.

  • Collaborates effectively with Bariatric Surgeon team to ensure patient is progressing with all pre-surgery requirements.

  • Communicates effectively with patients, families and caregivers building a trust relationship, mutual respect, and demonstrates empathy, integrity, honesty, compassion and cultural sensitivity in all interactions.

  • Empowers patients, families and caregivers to self-advocate and communicate well with medical team.

  • Upholds diversity, equity, and inclusion in all interactions and identifies barriers created through gaps in health literacy and language discordance.

  • Properly documents interventions in the electronic medical record and maintains data tracking as identified by supervisor.

Education Requirements:

● None required as long as other position qualifications are met

● Completion of a Medical Assisting program accredited by either the Commission on Accreditation of Allied Health Education Programs (CAAHEP) or the Accreditation Bureau of Health Education Schools (ABHES) is strongly preferred but not required.

Licensure/Certification Requirements:

● Requires Medical Assistant certification by either the American Association of Medical Assistants (AAMA), American Medical Technologists (AMT), National Health Career Association (CCMA), National Association of Health Professionals (NRCMA), or National Center for Competency Testing (NCMA).

  • Basic Life Support Certification from American Heart Association (AHA) or American Red Cross (ARC) required.

Professional Experience Requirements:

● Requires at least two years’ experience as a medical assistant with at least one year of experience working with Bariatric patients.

Knowledge/Skills/and Abilities Requirements:

● Possesses excellent written, verbal and interpersonal communication skills.

● Ability to work with detailed information and employ high-level problem-solving skills.

● Strong customer service and relationship-building skills.

● Strong communication skills with diverse populations.

● Ability to work independently but also collaborate with providers, staff, and external organizations.

● Demonstrates awareness and values cultural competency.

● Possesses excellent time management skills, ability to prioritize tasks and high work ethic.

● Exhibits a high degree of independence, adaptability, initiative, and commitment to the navigation team and its mission.

● Demonstrates an empathetic, compassionate and positive attitude.

● Proficient with Windows-based software including MS Word, Excel and Outlook.
