New Horizon Counseling Center Quality Assurance Specialist in Queens, New York

Job description

NHCC seeks a Quality Assurance Specialist in Queens for the Health Home Care Management Program. The QA Specialist utilizes independent discretion and judgment to assure accurate documentation compliance with the SDOH, Health Home, and OMH program regulations and assistance in program reporting.

Responsibilities

  • Collaborate with the Team to ensure the policies and procedures of the Health Home Care Management Program are followed.

  • Provide oversight and supportive training to Care Management staff on appropriate documentation and chart compliance, utilizing discretion and judgment.

  • Utilize the educational training, skills and experience obtained from advanced degree to provide appropriate professional services, techniques and strategies for clients under the direction of the Assistant Director and Clinical Director.

  • Ensure all forms and program documentation is in compliance with the program requirements by conducting regular chart audits and reviewing all documentation.

  • Assist team members in chart documentation when needed.

  • Implement self-audit tools and assist Assistant Director and Clinical Director in preparation for all program audits conducted.

  • Assist Assistant Director and Clinical Director in completing reporting requirements.

  • Work with administrative staff to ensure proper filing and storage of charts in compliance with regulations.

Position is Monday-Friday 9:00AM-5:00PM

Requirements:

  • Bachelor's Degree in social work, psychology, human services, and or public health. (Master's degree preferred).

  • Compliance/quality assurance experienced preferred.

  • Health Home Care Coordination experience preferred.

  • Excellent communication and organization skills.

  • Must be proficient in computer applications and other automated systems such as spreadsheets, Microsoft Office applications, calendar, and email and/or database software necessary to perform work.
