Rural Resources Community Action Care Coordinator in Pullman, Washington

Rural Resources Community Action is looking for a Care Coordinator for our Community Based Teams Division. This position is full-time (40 hours per week) with benefits and based out of our Pullman, WA office.

As a Care Coordinator, you will provide care coordination services to eligible individuals and their families, assesses needs, develop and monitor implementation of service plans, makes appropriate referrals and serves as a client advocate with other service providers. This position uses health promotion, wellness coaching and awareness of chronic health condition to support client's continued health improvement and help reduce utilization of emergency services and decrease recurrence of hospitalizations. Care coordinators meet their clients in their homes and in the community and provide one on one assistance and case management support.

A Care Coordinator will be well organized, and work independently to execute assigned programs, including documenting their work. The qualities we are looking for include ability to establish and maintain effective working relationships with staff, with partners outside organizations, and the general public, perform social and economic research about resources, and communicate effectively, both orally and in writing. Candidates should have knowledge of community-based human services principles, and modern business practices. Medical systems of care, medical terminology, and experience in the medical setting or lived medical experience preferred. Bonus points for multicultural awareness and English/Spanish bilingual abilities.

To apply for this position, you would need a Bachelor of Arts/Science degree in a human service related field, two years experience working for a human services agency, one year supervisory experience. Or any combination of education and experience which would provide the applicant with the desired skills, knowledge and abilities required to perform the job.

Requirements:

  • Access and egress non-ADA facilities and homes.
  • First Aid/CPR certificate
  • Must have reliable transportation.
  • Acceptable completion of a criminal history background check.

Salary & Benefit Information

The hiring range for this role is $21.65-$23.42 per hour. The full salary range of $21.65-$36.06 per hour represents the low and high end of the total salary range for this position. Actual salaries will vary depending on factors including but not limited to experience & qualifications. As a full or part-time employee, after the successful completion of our six-month training period you may receive a 4% step increase and an additional 4% step increase on your annual anniversary thereafter (excludes temporary employees). Depending on full time or part time status, other benefits may include medical (three plan options), dental and vision insurance, vacation, a 403(B) and/or Roth IRA retirement savings plan with a company match of up to 6%, $12,000 life insurance policy (while employed with RRCA), Employee Assistance Program and other auxiliary insurance options.  All employees, regardless of temporary, part-time or full-time status will receive sick leave.
