Meeting Street Service Coordinator in Providence, Rhode Island

Meeting Street is currently looking for a Service Coordinator for Early Intervention working 40 hours weekly.

Duties:

  • Provide home-based and remote case management to families with children ages 0-3 with developmental delays.
  • Collaborate with the family to develop the Individualized Family Service Plan and outcomes for family supports, strategies and relevant services to be provided by our multi-disciplinary team of therapists and educators.
  • Conduct initial and ongoing assessment of the childs development and functional skills, review progress, and outcomes to determine the progress made by each client while receiving early intervention services.
  • Identify and refer families to community resources that support IFSP outcomes, address immediate family needs, or support natural learning opportunities.
  • Ensure that all events and documentation are completed within the programs required timelines.

Requirements:

  • Bachelors Degree in Early Childhood Education, Child Development; Early Childhood Special Education, Social Work, Psychology, Communication Disorders, Nutrition or a related EI field.
  • Bachelors Degree in a non-related field and 3+ years experience working with or caring for infants and toddlers with special health care needs and their families.
  • Or a High School diploma and 6+ years relevant experience with approval from EOHHS.
