Norbella Senior Living of Prior Lake is looking for a part-time LPN! Tuition Discount Opportunities/ Scholarships/ Student Loan Forgiveness (site specific, ask for details) LPN Job Objective(s): The role of the LPN is to promote the health and safety of all residents through on-site visits with the residents and staff. The LPN will ensure staff are trained in current medical procedures and policies. Display a positive work attitude and be able to work as a team with other staff members. LPN Responsibilities: Assures residents maintainadmission criteria, monitor, and recommend alternative placement if necessary. Assures residents have up to date care plans per state regulations Monitors and maintains resident charts and clinical progress notes. Communicates to physicians, staff and family changes in resident's needs or conditions. Facilitates coordination of resident services with other service providers. Oversees medications and renewals and any necessary follow-up with physicians about medications and/or medical conditions. Assures that medication administration procedures are followed properly and discusses any needed corrections, errors, etc., with staff. Communicates in resident's log any information that staff need to be aware of. Provides alternating supervisory visits in coordination with the RN. Works as a team player with all members of the residents' care team. Supervises medication practices and medical equipment being used in the building. Assures that staff clearly understand and follow all resident care procedures. Shares resident and family concerns with pertinent staff if appropriate. Works with manager to assure emergency procedures are in place LPN Qualifications: Maintains a current state license as a Licensed Practical Nurse per state regulations A minimum of one year's experience in home health, assisted living or long-term care is preferred Good organizational skills with the ability to self-start and follow through with initiatives with a minimal amount of supervision Exhibits a caring heart with a passion to care for the elderly Great Lakes Management Company is an at-will employer; this job description is a guideline and does not constitute a written or implied employment contract.
