Geriatric Social Worker

EAC Network, a not-for-profit social service agency that empowers, assists, and cares for over 62,888 people in need through 100 programs across Long Island and NYC, seeks a Geriatric Social Worker to work full time for its Geriatric Social Work Services program.

The Geriatric Social Worker position is a non-exempt position paying $25-30 per hour at 35 hours per week. The hours of operation are Monday-Friday 9:00AM-4:30PM and the office is located in Port Washington, NY. The role reports to the Senior Director.

Primary Purpose of Job:

The Geriatric Social Worker provides social work services including a comprehensive psychosocial assessment and care plan, individual and group counseling, case management and referral services for Town of North Hempstead residents 60 years of age and over.

Principal Duties & Responsibilities:

  • Provide a comprehensive psychosocial assessment and develop a care plan to address unmet needs of older Town of North Hempstead residents to help them live safely in their own homes.

  • Complete screening for benefit/entitlement programs and assist with completing applications as needed.

  • Identify the need for additional services and make appropriate referrals.

  • Provide individual and group counseling sessions.

  • Become proficient in the use of 311/Townstat service request system.

  • Develop & maintain client records, statistics and written reports in accordance with HIPAA guidelines & Project Independence requirements.

  • Attend and report to the regional community advisory committee(s) & assist with facilitating meetings.

  • Develop relationships with community services providers & provide education on Project Independence at libraries, schools, faith-based organizations, local merchant & senior citizen groups.

  • Assist the Department of Services for the Aging (DOSA) staff with community outreach for Project Independence.

    Knowledge, Skills, & Abilities Required:

  • Masters degree in social work from an accredited school of Social Work required or Gerontology degree may be considered.

  • Must possess knowledge of Nassau County services and needs of the Veteran and Geriatric population.

  • Ability to communicate with and understand the needs of older Town of N. Hempstead residents.

  • Must be proficient in computer applications or other automated systems such as spreadsheets, Microsoft Office applications, calendar, and email and/or database software and ability to utilize a laptop computer necessary to perform work assignments.

  • Ability to speak at and/or facilitate meetings/seminars with large groups

  • Strong verbal and written communication skills

  • Fluency in English required, bi-lingual, Spanish, French/Creole, Farsi a plus

  • Must have access to automobile and possess valid NYS Driver's License and ability to drive to multiple sites within the Town of North Hempstead.
