McLaren Health Care Clinical Supervisor Care Manager - Port Huron - McLaren Careers in Port Huron, Michigan

Position Summary:

Responsible for the management of cost efficient care to home health patients ensuring quality delivery of outcomes based on care that meets the individual needs of the patients/families. Responsible for identification of educational opportunities from performance improvement and record review activities to ensure quality processes and outcomes are implemented in the Home Care Quality Plan for the Program.

Essential Functions and Responsibilities:

  1. Assists with evaluation of the clinical team and provides on-the-job training in deficient areas.

  2. Evaluates the provision of patient care and accuracy of supporting documentation through patient clinical record audits including but not limited to; OASIS, ICD-10 coding, Plans of Care, initial physician orders.

  3. Monitors staff performance related to OASIS documentation.

  4. Assists, prepares and presents at operations meetings, and submits required reports in a professional and timely manner.

  5. Assumes on call responsibilities on a rotational basis.

  6. Translates and interprets policies and procedures that guide and support the provision of services of the agency.

Required:

Current license to practice as a Registered Nurse in the State of Michigan

Minimum of two years Home Health care or Hospice experience

Valid driver’s license and proof of valid auto insurance

Preferred:

Bachelor of Science in Nursing Degree (BSN)

One year of management experience

Experience with homecare regulatory and accrediting bodies

Supervisory experience.
