PA Inclusive Program Supervisor/Case Manager in Pittston, Pennsylvania

PA Inclusive is an agency that believes people with disabilities should live where, with who, and in what they want and that those who want to work can work in an area of their interest in a 'typical' job in the community. Work and living support services can and should be delivered to support them how they choose. This program oversees the OVR and ODP IDD Waiver programs offered through PA Inclusive.

 

Description:

The Case Manager/Supervisor role includes overseeing disability services that focus on inclusion for people with disabilities. These services could include ODP IDD Waiver programs, such as In-Home and Community Supports, Community Participation Supports, Supported Employment, Supported Living, and Housing Tenancy and Sustaining Services (as well as others in the IDD Waiver system that we may add in the future), as well as OVR PETS and Adult Supported Employment services. These services help people who live at home, in the community, and in their own apartments, including promoting independence, working on personal goals, helping the individual participate in community activities and general in-home daily tasks, as well as finding and keeping jobs in competitive, integrated, and customizable employment.

 

Job Duties:

  • Oversee strategic delivery of employment and community-based services through OVR and OPD ID Waiver systems.
  • Management of staff that ensures our community members' safety, health, and well-being.
  • Supervise program staff under your direction. Hire, train, and supervise staff as needed.
  • Ensure the utilization of patient-directed services as needed and perform check-ins regularly.
  • Manage compliance matters related directly to programs, such as managing client waiver files.
  • Ensure supervision of programs regarding backup plans, 24-7 coverage, and fill in staff so that those we support always have staff when required.
  • Serve as agency liaison to outside entities regarding clients in programs, such as receiving referrals and communicating with ODP and Support Coordinators. Attend ISPs, Intake Meetings, and other meetings as necessary.
  • Monitor budgets and program-assigned funds.
  • Develop, monitor, and evaluate programs in compliance with the current regulations.
  • Work collectively with billing, training, QA, Compliance, and administration to effectively deliver services and collectively ensure compliance at all times.
  • Develop strong relationships with community agencies, businesses, community leaders, and neighborhood-based organizations and develop additional program resources when appropriate.
  • Attend agency meetings as requested; host agency-related meetings as appropriate.
  • Attend external events for the purpose of program development.
  • Person must be willing to work flexible hours and be on call 24-7.
  • This position requires a minimum of 35% time allocation in the field providing direct service.
  • Serve as a member of the leadership team of the agency.
  • Other related duties as assigned.

Qualifications

  • BA degree required; MS Degree, LMSSW preferred.
  • Experience in direct client-facing service delivery in a human service setting; a minimum of three years of management experience in a human service setting is required.
  • Experience working with adults with disabilities, autism, or mental health disabilities in employment settings preferred.
  • Experience in housing-based services.
  • Demonstrated achievement through supervisory, staff development, program management, an budgetary skills.
  • Excellent interpersonal and verbal communication skills.
  • Computer literacy, particularly with MS applications.
  • Ability to obtain state and agency-required employment certifications including, but not limited to, NADD CBDSP, CPS, PREP, Harry M, ACRE, and/or Discovery within 3 months of employment.
  • Position requires effective communication and presentation skills, computer knowledge, strong documentation, and organizational skills.
  • Valid Driver's License and Clean Driving Record.
  • Must Pass Child Abuse History and Criminal Record Check Clearances and not be excluded from State of PA Exclusion Lists.
