ServiceNet is looking for an experienced, forward-thinking case manager to join our team in our Shelter and Housing Division. Together, we provide our guests with the time and support they need to get their lives back on track and make a successful transition into permanent housing. Position Responsibilities: Conduct intakes and assessments for guests as they enter the program; Provide support and advocacy to shelter and transitional housing recovery participants assisting them to access, utilize, and follow through with treatment, social service, medical, vocational, housing, and associated community resources that promote self-sufficiency. Weekly meetings with families / individuals to review service treatment plans, set goals, facilitate referrals, and evaluate progress. Contribute to the creation of a positive residential milieu in collaboration with participants and shelter staff. Write clear and objective case management notes, progress notes, and any other record keeping that may be assigned. Qualifications: Bilingual (Haitian Creole / French / Spanish) candidates encouraged to apply! High school diploma or equivalent, plus additional training equal to two years of college. One year of experience working with people living on a low income, or who are affected by mental health issues and/or addiction. Knowledge of surrounding area social service agencies. Valid Driver\'s license and an acceptable motor vehicle record. Benefits / Incentives: Starting salary \$40,000/yr. Paid orientation and trainings. Generous time-off package.
