Devereux Advanced Behavioral Health Arizonas Community Based Servicesprogram provides in-home remedial intervention for at-risk youth and families identifying youth and family strengths and needs to enhance the Child and Family Team (CFT) planning process. We work with families and youths in their homes, teaching a variety of skills ranging from self-care, household management, living, and social. We are looking for talented individuals who have experience working in Behavioral Health, with families and at-risk youth, to be a positive role model and impact on our families!

Reporting to the Program Manager, theFamily Support Coordinator (Crisis Support Coordinator)is responsible for the coordination of care, treatment planning, service delivery, and case management of our placement preservation and crisis stabilization services. This position formulates treatment plans, assessments, clinical impression, and discharge planning. The Crisis Support Coordinator also provides support to individuals, families, and placements to help stabilize behaviors and avoid disruption. You will work closely with the Child/Family Team to identify the supports needed to ensure stabilization and maintain stability successfully moving forward.
