What You'll Do:                                                 

The Bilingual (Spanish) Certified Diabetes Care and Education Specialist (CDCES)/Registered Dietitian (RD) or Registered Nurse (RN)  will work directly with individuals to provide diabetes education/support,  intensive nutrition counseling, and weight management education/support with the goal of improving health outcomes. This remote position is responsible for consistently meeting program metrics while also meeting quality requirements. The CDCES/RD-RN position is a part-time and independent contractor role. 

 

Primary Responsibilities:

  • Provide diabetes education/support, intensive nutrition counseling, and weight management education/support to members via phone, video, or digital (e.g. chat, text, email) on behalf of our customers to improve health outcomes.

  • Provide medication management and care for diabetes and related comorbidities under the supervision of the assigned physician.

  • Provide remote patient monitoring for devices (e.g. scale, BG meter, BP monitor, Ketone meter, CGM etc.).

  • Provide support, including dosing and titration within established protocol, to individuals prescribed diabetes medication.

  • Interact with team physicians and external providers.

  • Quickly establish rapport and therapeutic alliances with a diverse population of members.

  • Support behavior change among patients using motivational interviewing techniques.

  • Present online webinars if needed.

  • Consistently meet program metrics while also meeting quality requirements.

  • Other duties as assigned.

     

    Requirements and Experience:

  • Must have current certification as a Certified Diabetes Care and Education Specialist from the National Certification Board for Diabetes Educators (NCBDE).

  • Must currently be a Registered Dietitian credentialled by the Commission of Dietetic Registration. Or must be a licensed Registered Nurse in the state you reside.

  • Must be licensed if required by the state you live in.

  • Minimum 2 years of clinical experience in the area of diabetes care.

  • Must be fluent in English and Spanish.

  • Experience supporting and educating on diabetes medications is preferred.

  • Proven experience utilizing motivational interviewing to provide behavior change.

  • Demonstrated ability to receive constructive feedback and then incorporate it into member calls to drive performance.

  • Must have a home office for private patient conversations along with high-speed internet.

  • Able to work independently and manage time efficiently.

  • Must be comfortable setting up and maintaining company-issued equipment (e.g. laptop, monitor, etc.).

  • Must be comfortable utilizing advanced technology (electronic medical records, Microsoft Office, scripting applications, device data, etc.).

  • Demonstrated ability to recognize patient needs and adapt communication techniques to best serve a diverse patient population.

  • Must have a flexible schedule and be able to commit to a minimum of 20 hours per week to accommodate members.

  • Cannot hold full-time employment elsewhere.

  • Must live in the United States.
