Reference #: 9302342 PRIMARY FUNCTION: Provide clinical support to the Jefferson Sleep Disorders Center Team. ESSENTIAL FUNCTIONS Assessment, diagnosis and case management of patients in the outpatient clinic and inpatient consultation services of the Sleep Disorders Center (SDC) in conjunction with the Collaborating Physician. Maintaining continuity of care for outpatient and hospitalized patients. Conducting evaluations for new and established outpatients and inpatients and ordering consultations and laboratory studies accordingly. Ordering sleep laboratory studies and explaining results and indicated treatments to patients. Conferring with referring clinicians and other practitioners, formulating ongoing reports. Conducting treatment for sleep disorders with a variety of methods, including CPAP, BiPAP, noninvasive pulmonary ventilation, cognitive behavioral therapy, behavioral weight loss, dental appliances, medications, phototherapy, sleep hygiene education, oxygen administration, circadian rhythm adjustment, among others Communicating findings, diagnostic impressions, and treatment plans to outpatients and inpatient in person and by telephone and to referring physicians by telephone and written communications. Documentation of patient interactions in the outpatient and inpatient charting systems Enhancing compliance for patients receiving positive airway therapy, non-invasive ventilation, and other treatment modalities and collaborating with respiratory therapist, nurses, and other members of the multidisciplinary health team to help meet patient care goals Conferring with members of inpatient consultation team, including physicians, respiratory therapists, technicians, and others, and working with them in a coordinated fashion. Providing education to physician, medical student, and other trainees on service. Developing and providing patient education related to sleep disorders on a one-on-one basis, as well as by organized conferences and community activities Interacts with co-workers, visitors, and other staff consistent with the core values of the University. Masterrsquo;s Degree National Board Certification as an Acute Care Nurse Practitioner or Physician Assistant BLS certification Two yearrsquo;s experience as a registered nurse is required, preferably in pulmonary, sleep or related area. Written and verbal skills in English language are required. Knowledge of clinical and organization skills/techniques and knowledge of hospital ancillary department functions generally acquired through previous education and/or experience in a related field is required.
