Job Summary Care coordination (outpatient and inpatient), Quality Improvement Initiatives, Conferences and Meetings, Administrative Responsibilities. mandated by the National Cystic Fibrosis (CF) Foundation and required for accreditation of a Cystic Fibrosis Center. Essential Functions CARE COORDINATION (OUTPATIENT AND INPATIENT) Perform nutritional assessments on all individuals with CF, including monitoring growth, reviewing/ monitoring nutritional requirements and symptoms of malabsorption. Review medical charts during chart review, and as needed, for completion of records and annual data, and other pertinent laboratory tests. Provides nutritional education for all individuals with CF and their families. Establish goals and plan nutritional care; communicates plan to patient, family and staff.  Adjust care plans with attending physician as needed. Counsel patients and families in normal and therapeutic nutrition.  Tailor dietary instructions to personal family needs. Contact patients regarding lab results and recommendations (after review with the CF team/ physician.) Completes any statements of medical necessity as applies to nutritional issues. Administrative duties including coordination of oral nutrition supplements and enteral feeding supplies(i.e. letter of medical necessity, notes, scripts) Along with the social worker, assess for food insecurity at each clinic visit. Discuss food insecurity issues with CF team. Connect patients/families with appropriate resources specific to their needs and location. QUALITY IMPROVEMENT INITIATIVES (QI) Participate in Quality Improvement Initiatives. -Participate in clinical research when appropriate. Assessing and addressing FI -Continue Food insecurity QI project- collect data and reassess quarterly. CONFERENCES AND MEETINGS- as outlined by the CFF and CF Center Director Participate in weekly pre-planning outpatient conference and weekly inpatient rounds. Be a functioning multidisciplinary team member, regularly attending and contributing to pre-clinic meetings. Attend/ participate in weekly CF Team meetings. Be a functioning multidisciplinary team member, regularly attending and contributing to meetings. ADMINISTRATIVE RESPONSIBILITIES/ PROFESSIONAL DEVELOPMENT: Education Opportunity to attend and participate in conferences to maintain contact with advances in nutrition and diet therapy, including in relation to CF. Attend North American Cystic Fibrosis Conference at the discretion of the Center Director Attend Mid-Atlantic Cystic Fibrosis Consortium Conference SW and Dietitian Consortium Participate in CF Dietitian Listserve. Participate in planning and executing Family Education events. Participates in teaching in the Section of Pulmonology. Maintains/ has ongoing relationship with the CF Foundation, including local chapter, and other organizations. Liaison between CF Center and companies associated with nutritional aspects of care (i.e.formula companies, PERT and CF vitamins). Develop educational materials with regards to nutrition in CF  - Update CF Center Resources manual related to nutrition as new resources are identified; annually review CF Center Resources manual. Required Qualifications Minimum of a Bachelor's Degree in Nutrition, Registered Dietitian (RD) or Equivalent Minimum of 2 years of experience. Credentialed from the Committee on Dietetic Registration (CDR) Preferred Qualifications Licensure in the state of Pennsylvania (LDN). Physical Requirements Typically sitting at a desk/table. Typically standing, walking. Location Philadelphia, PA Additional Information This position is classified as Non Exempt, grade K. Compensation for this grade ranges from  $26.16 to  $ 39.24 per hour.
