Confidential Employer Case Manager - Children in Phila, Pennsylvania

Blended case management is a service which will assisteligible individuals with mental illness, including children with a seriousmental illness or emotional disorder, in gaining access to needed medical,social, educational, and other services.  

Duties/Responsibilities: 1. Assist the consumer or their family in obtaining and maintainingculturally appropriate basic living needs and skills such a: housing, food,medical care, recreation, education and employment. 2. Provide services within the context of the consumer and their family'sculture. 3. Provide case management service in accordance with a written,consumer-specific service plan which assesses strengths, needs interestsand is goal and outcome oriented. 4. Provide case management services as needed in the place where theconsumer/family resides or needs the service. Services may also be provided in the Case Management office when off-site interventions wouldnot be appropriate. 5. Contact the consumer or their family will be determined by anassessment of their need. Face-to-face contact with the consumer shall bemade on the basis of the assessment and will range from once a week toone time per month. 6. Ensure that when a consumer or their family that cannot be contactedduring any visit that they are provided immediate follow-up and that thiscontinues until the consumer's status is clarified. 7. Provide crisis intervention services to all children and adolescent CaseManagement consumers. 8. Written case notes should document the purpose for each contact andreflect the goals and objectives of the service plan. 9. Complete all required paperwork and submit such paperwork by stateddeadlines. 10. Maintain Case Management records and ensure that alldocumentation is included in the consumer's records. 11. Provide continuity of care for children and adolescents receiving CaseManagement services who are hospitalized. 12. Will provide telehealth services when necessary to the consumer andtheir family or any other pertaining parties that is part of the consumer'scase.

Education and Experience: · A bachelor's degree with major course work in sociology, social work,psychology, gerontology, anthropology, political science, history, criminaljustice, theology, counseling, education. PLUS a minimum of one (1) year experience (paid or unpaid) in a humanservice field. This experience must involve direct contact with theindividual receiving services (i.e. coaching, teaching, case management,etc.) OR · A high school diploma and twelve (12) semester credit hours in sociology,social welfare, psychology, gerontology or other social science. PLUS two (2) years paid experience in public or private human serviceswith one (1) year in direct consumer contact. · All Case Managers must also show proof of a valid driver's license andcurrent insurance with minimum limits of$100,000 per occurrence and $300,000 aggregate. · Must have a registered operable Automobile. Preferred: Case Management Certification through BHTEN Mandatory Requirements: · FBI Criminal Background Clearance · Pennsylvania Child Abuse Clearance · PA Criminal Check · CPR/First Aid certification
