Responsible for the delivery of nursing care within the assigned unit scope of practice following organizational policies and procedures, nursing process guidelines, standards of care, and standards of practice.

  • Performs entire Annual Wellness Visit (AWV) appointment directly with patients:

  • Maintains AWV criteria and obtains updates from Center for Medicare and Medicaid Services (CMS).

  • Provides reimbursable RN services.

  • Conducts in-depth verbal interviews with elderly patients to obtain health sensitive information.

  • Collects patient history.

Required:

  • Graduate of an accredited school of nursing.

  • Current State of Michigan Registered Nurse license.

  • For clinics with the Rural Health Designation, Basic Life Support (BLS) issued by the American Heart Association (AHA), American Red Cross (ARC), and Canadian equivalents, (MHC approved providers for the resuscitation certification) is required within 6 months of hire Preferred:

  • ACLS certification
