Job Overview

Serves as the point of contact for referring physicians, patients/families and caregivers to provide resources and assistance with accessing clinical, supportive care and financial services offered and available in the St. Joseph Healthcare System and community. Coordinates activities of the multidisciplinary team to ensure coordination of services and continuity of care for the patient. Anticipates barriers to care and services and intervenes to ensure the continuity of efficient/timely care. Facilitates transitions of care to assure that patients have appropriate referrals and follow-up appointments. Delivers and coordinates patient education to ensure quality outcomes including the patient's compliance with plan of care.

Qualifications

Knowledge of theories, principles, and concepts normally acquired through completion of a Bachelor's degree in Nursing, and three to five years of previous work related experience. Licensure required as a Registered Nurse by the State of New Jersey. Successful completion of a department specific orientation and department specific certification of Basic Life Support (BLS) within one year of hire. Knowledge of Microsoft Excel and Word plus proficient data entry skills are necessary.

St. Joseph’s Health requires all employee certifications for Basic Life Support (BLS), Advanced Cardiovascular Life Support (ACLS) and Pediatric Advanced Life Support (PALS) to be granted through the American Heart Association
