Case Manager Rapid Rehousing

Job Summary:

You will provide housing search and procurement and case management services to Bergen County families and individuals who are homeless as defined by U.S. Department of Housing and Urban Development (HUD).

What You Will Do:

  • Assist participants with housing search, procurement, and landlord negotiations.

  • Provide a range of services varying from on-site intake and assessments to community case management services, following linkage to permanent housing.

  • Assess clients at intake, during ongoing service delivery and at discharge

  • Complete ongoing evaluations/reassessments through regular client contact to appraise changing needs and progress toward goals. Such contact should consist of home visits, client telephone conferences, client office visits, and visits to other community locations

  • Formulate comprehensive treatment plans containing relevant goals, measurable objectives and realistic time frames for achievement, and perform reviews of progress and evaluate the effectiveness of goals/objectives

  • Gather client's financial documents and complete quarterly renewal calculation packets

  • Promote the maintenance of permanent housing, independence and stability in the community

  • Engage clients in services and address their needs; provide linkage to health care services, dental care, benefits, mental health services, substance abuse services, family consultations crisis intervention and vocational and/or education and/or employment services etc.

  • Participates in case collaboration meetings with supervisors, collaborating agencies, landlords, etc.

What We Need to See:

  • Bachelor's with 1-year experience working with the homeless population.

  • Experience in one of the following: crisis intervention, counseling, mental health, substance abuse, domestic violence, housing procurement, or job development

  • Must demonstrate good organizational, communication, negotiating, and problem-solving skills.
