Job Description :

  • Consistently attends to health needs of all individuals.
  • Institutes physician's orders as appropriate and advises Residential Nurse of new orders for medications and/or treatment.
  • Prepares, administers and documents all medications and treatments prescribed by physician's orders, promoting independence based on self-medication administration assessments and habilitation plans.
  • Administers insulin and monitors blood glucose levels via glucometer, administers injectables and medications via tube (g/j/PEG) as well as administering and ensuring accountability of controlled medications. Documents as required.
  • Follows standard procedure for ordering and receiving medications. Documents using proper forms.
  • Implements catheterization orders, tracheostomy care, pharyngeal suctioning as well as any additional medical nursing treatment as deemed necessary and within LPN standards of practice.
  • Documents health concerns (including seizures, menses, vital signs including temperatures, Client, unusual behavior, etc.) which may indicate health care problems; gathers facts and reports significant information to the appropriate health care providers. Attends medical appointments as assigned.
  • Assists staff with concerns or questions regarding individuals' care within the LPN scope of practice.
  • Participates in the development and review of habilitation plans/goal plans. Implements and provides documentation for plans.
  • Provides opportunities for Community inclusion utilizing community resources to develop personal relationships with members of the community.
  • Provides opportunities to make appropriate choices; offers suggestions for new choices.
  • Provides training and assistance to develop skills to promote self-reliance.
  • Provides opportunities for individuals to contribute to his/her own life and the lives of those around them, both in their community and their home.
  • Ensures timely documentation of activities in individual's habilitation/medical notes.
  • Promotes individuals' safety and rights on a daily basis.
  • Treats individuals with respect and dignity, observing their rights to privacy, confidentiality, and personal choice.
  • Provides individuals with the level of supervision which meets their needs as identified in their Protective Oversight Plan (ex: fire drill evacuation, bedtime safety, etc.) and/or specific issues that may arise. (ex: illness)
  • Ensures safety by reporting needed repairs/safety issues in a timely manner.
  • Reports all allegations of neglect, mistreatment, and abuse, promptly and according to established guidelines.
  • Maintains current LPN licensure, CPR certification, and mandated Infection Control course work and training.
  • Consistently adheres to specific duties which are necessary to assure smooth program operations.
  • Participates in proper meal preparation & portion control according to prescribed diets & individuals' preferences. Ensures proper food storage following proper sanitation principles.
  • Maintains a clean and homelike environment.
  • Follows Personnel and Medical policies and procedures; completes necessary documentation (ex: Time and Attendance records, shift reports, minor event logs, etc.)
  • Works cooperatively with and communicates appropriately with other team members, promoting a harmonious environment for the consumer.
Performs LPN Duties as Needed:
  • Administers medications and injections.
  • Performs tube feedings.
  • Collects blood samples if certified for venipuncture.
  • Provides colostomy care.
  • Observes individuals and reports changes in their condition.
  • Takes and records temperature, pulse, respiration and blood pressure.
  • Obtains specimens for analysis and performs other medical procedures as ordered by a physician.
  • Prepares and applies simple dressings.
  • Bathes and feeds patients/residents.
  • Cares for individuals and their quarters by performing such tasks as making beds, changing linen, lifting and moving consumers and caring for their personal effects.
Documents Required:
  • Active LPN License.
  • BLS/CPR issued by AHA.
  • Covid Card.
  • Updated Resume.
