Schedule: Monday- Friday 8 am- 5 pm; rotating on-call after hours

A Brief Overview

Provides leadership, direction and supervision for all Home Health Pharmacy related activities in order to accomplish organizational and departmental strategic goals and objectives. Demonstrates mastery of knowledge and skills required in assessing, planning, implementing and evaluating complex care needs of pediatric patients with acute and chronic disorders. Responsible for all day-to-day operations of the department.

Essential Functions

  • Develops and implements goals, objectives, plans, policies and procedures to achieve desired departmental outcomes as established in conjunction with the pharmacy and home health leadership teams. • Actively participates in researching, developing, and planning new service opportunities. • Oversees the planning, implementation and evaluation of approved projects. • Identifies, obtains and appropriately utilizes resources necessary to meet the identified goals including recommending budget amounts for operating expenses and capital purchases. • Collects, organizes and submits necessary workload statistics and financial data • Ensures proper pharmaceutical care and practice including but not limited to monitoring/evaluating/implementing prescription drug orders, dispensing prescription drug and device orders, educating patients on the proper use or delivery of medication, completing drug regimen reviews, providing patient counseling, and medication therapy management • Responsible for compounding and labeling of drugs and devices and proper and safe storage of drugs and devices. • Ensures compliance with federal and state licensure requirements for pharmacy staff and that pharmacy operates in accordance with all federal and state statutes and regulations pertaining to pharmacy. • Oversees pharmacy staff and operations, including but not limited to record-keeping and auditing, inventory, pharmacy maintenance, and core pharmacy workflow. Assist with staffing as needed to ensure patient care needs are met. • Maintains working knowledge of and compliance with the HRSA 340B Program • Develops and complies with processes for addressing medication recalls, employee complaints and appropriate follow-up and documentation • Puts measures in place to mitigate diversion, controlled substances or otherwise • Fulfills all responsibilities as designated Pharmacist-in-Charge as required for pharmacy licensure.

  • Responsible for establishing staff levels needed to meet the needs of the department and the patients served. • Selects/hires employees and trains and/or oversees the training on job related tasks. • Conducts employee performance management duties including completion and delivery of performance evaluations, setting goals, dissemination of information, coaching, addressing performance issues/routine correction actions, and recommending termination of employment. • Makes salary recommendations. • Submits notifications for personnel actions (e.g. status changes, terminations).

  • Coordinates all services provided by the department. Assures that the needs of the customers are met. • Ensures that patient care needs are assessed and communicated with members of the healthcare team. • Orders and maintains inventory supplies and services for the provision of pharmacy services • Reviews and authorizes payment of invoices, ensuring proper pricing and terms. • Communicates with the pharmacy leadership team regarding pharmaceutical shortages, changes in products, deleted products and pricing updates. • Assists with overall development and implementation of the Specialty Pharmacy Performance Improvement Plan • Assists in the identification of goals and related client/patient outcomes as part of the Specialty Pharmacy Performance Improvement Plan • Coordinates, participates in, and reports activities and outcomes associated with Specialty Pharmacy Performance Improvement • Maintains compliance with Medication Management sections of Joint Commission requirements • Ensures all aspects of USP 795 and 797 are maintained and compliant

  • Responsible for the timely and accurate dispensing of pharmaceuticals, ensuring proper preparation, labeling, dispensing and billing. • Reviews medication orders and appropriately triages urgent, routine, and PRN medication orders • Understands and demonstrates appropriate order entry, preparation and documentation of the following pharmaceutical products. • Appropriately reviews medication therapy and intervenes with healthcare team as warranted. • Evaluates medication order for appropriate medication selection, dosage, route, frequency, duration of therapy and drug interactions (drug-drug, allergy, drug-disease, drug-lab, drug-food) in relation to the patient’s clinical status. • Performs daily profile review to assess appropriateness of medication therapy as detailed in “a” in relation to patient’s ongoing and changing disease state and clinical status.

  • Ensures availability 24 hours a day, 7 days a week.

  • Assumes after hour call as needed.

Education Qualifications

  • PharmD from accredited college or university Required

Experience Qualifications

  • 1-3 years experience, including 1 year in a supervisory role Required and

  • 1-3 years USP 795 and USP 797 Experience Preferred and

  • 1-3 years Specialty Pharmacy Accreditation Experience Preferred

Skills and Abilities

  • Knowledge of pediatric pharmaceutical, home health, and specialty pharmacy care (Medium proficiency)

  • Excellent oral, written and communication skills (High proficiency)

  • Ability to plan, direct and evaluate the work of pharmacy personnel. (High proficiency)

  • Ability to communicate effectively with individuals and groups in a wide variety of settings. (High proficiency)

Licenses and Certifications

  • RPH - Registered Pharmacist in the state of Nebraska Upon Hire Required and

  • BLS - Basic Life Support through the American Heart Association Upon Hire Required

  • PHARMD - Licensed Pharmacist Doctor of Pharmacy Degree Upon Hire Required
