Essential Job Functions:

  • Support the implementation of unified systems of care, guiding families toward long-term stability.
  • Provide coaching, modeling, and support to empower individuals and families, helping them find strength and resources.
  • Respect the integrity and confidentiality of individuals receiving services.
  • Foster the development and strengthening of natural supports within the community.
  • Actively participate in treatment with child, youth, and family teams, aiming to keep them in the home and avoid out-of-home placements.
  • Empower individuals and families through modeling, advocacy, coaching, and support.
  • Develop and maintain positive relationships with community partners, serving as a liaison for NorthCare.
  • Manage complex cases, coordinating services through the wraparound process.
  • Maintain accurate records and documentation in compliance with requirements.
  • Meet patient contact/caseload standards as established by the Director.
  • Ensure prompt communication via phone, text, and email.
  • Maintain consistent work attendance and promptly communicate any late arrivals or absences.
  • Participate in meetings, consultations, and training sessions to expand your knowledge.
  • Provide services in the community, including clients\' homes, local venues, or via telehealth.
  • Transport patients to various locations in the community and provide services in the home.
