JOB REQUIREMENTS: Our nursing staff play a key role on our team to serve the residents and ensure their good health. We are proud of our Five Star rating on Medicare\'s Care Compare and you can be assured we strive to exceed expectations. You will work closely with other nursing professionals to assure continuity of care from shift to shift. You may work with rehab or long-term care residents. Current openings include benefit eligible positions, 4 shifts/week, on Day or PM shift. We also have part-time positions on PMs, 3 shifts/week, or every other weekend on Day or Night shift. (RN) Assess residents and identify problems and formulate resident goals and nursing intervention (Nursing Process) in I-Care Plan format. Report changes in resident condition to immediate supervisor. Call or accept verbal orders from resident\'s physician and transcribe them according to procedures and policies; co-sign Licensed Practical Nurse orders for all verbal or telephone orders. Document appropriately and report any drug and/or treatment incidents to Supervisor and/or physician. Carry out resident treatments according to physician orders. Initiate and update total plans of care and problem list for the resident in the computer. Supervise Nursing Assistants to assure quality resident care. Document and counsel staff when behavior or actions are inappropriate. Promote teamwork and culture change to improve resident quality of life. (LPN) Provide resident care in accordance with Shorehaven\'s policies and procedures, and State and Federal regulations. Receive report from departing shift and convey pertinent details to oncoming shift to ensure quality and continuity of care. Observe and report any changes of resident conditions to the RN in charge. Administer medications and treatments according to physician\'s orders. Supervise and evaluate the direct resident care given by Nurse Assistants; giving and receiving report from Nurse Assistants.
