JOB REQUIREMENTS: Restorative Nurse/House Charge Our Mission: Reflecting Christ\'s love, we provide homes and resources for seniors. Shorehaven has been serving seniors since 1939 and has been consistently rated as a Five-Star Facility on Medicare.gov! We are truly Specialists in Serving Seniors. Grounded in our faith-based mission, we provide compassionate care across a range of services from independent to skilled care. We are located on the west side of Oconomowoc, on the shores of Lac La Belle. We are seeking an experienced RN to provide Restorative Care for residents of our skilled nursing facility and act as Charge Nurse every other weekend. This position is scheduled three days per week (including every other weekend). Weekday schedule is flexible, weekend schedule is 6 a.m. - 2:15 p.m. The Restorative Nurse/House Charge is responsible for the following: Develop and maintain Restorative programs that meet the needs of the population in the facility. Work with Staff Development to ensure staff are trained in the techniques of the programs. Assist the QAPI Team in developing, implementing and maintaining an ongoing quality assurance program for Restorative programs. Participate in discharge planning from Restorative Programs. Assess residents, identify problems and formulate resident goals and nursing intervention. Report change in resident condition to attending physician and family. Identify emotional and social needs of residents and coordinate plans of care with appropriate department such as Social Services, Chaplaincy or Activities. Review medical orders and plans of care with physician and discuss changes based on resident conditions. Assist in training, supervising, counseling and evaluating assigned staff. What You Will Need to be Successful: Registered Nurse currently licensed by the State of Wisconsin with Geriatric Experience. Outstanding assessment skills Strong communication skills Restorative experience and certification is desirable. We offer a competitive starting salary, which is negotiable based on experience.
