Licensed Practical Nurse (LPN) -- Oak Harbor & Perrysburg

Full-Time Position -- Day Shift Monday thru Friday, Must be willing to travel

Job Summary -- We are looking for a compassionate and committed Licensed Practical Nurse (LPN) to become a valued member of our team. In this role, the LPN will deliver high-quality nursing care to our individuals under the guidance of a Registered Nurse (RN). The ideal candidate will possess strong clinical expertise, outstanding communication skills, and a deep passion for working with individuals with intellectual and developmental disabilities.

Key Responsibilities:

  • Administer medications and treatments as prescribed by physicians.
  • Monitor patients' vital signs and report any changes to the healthcare team.
  • Assist with patient assessments and the development of care plans.
  • Provide wound care, including dressing changes and monitoring for signs of infection.
  • Support patients with daily activities, such as bathing, dressing, and mobility.
  • Document patient care and progress accurately in medical records.
  • Collaborate with RNs, physicians, and other healthcare professionals to ensure optimal patient outcomes.
  • Educate patients and their families on health conditions, medications, and home care instructions.
  • Maintain a safe and clean environment in compliance with healthcare regulations.
  • Participate in continuing education and professional development activities.

Qualifications:

  • Current and valid LPN license in the state of Ohio.
  • Graduation from an accredited LPN program.
  • Minimum of 3 years of clinical nursing experience preferred.
  • Excellent communication and interpersonal skills.
  • Ability to work effectively in a team-oriented environment.
  • Strong organizational skills and attention to detail.
  • Compassionate, patient-focused attitude.
